She fears cancer. The abdomen is normal, the vagina very patulous, the uterine opening large, the cervix being deeply lacerated, the faces chronically granular, the tissue hyperplastic. The uterus is heavy, the canal measures 3 inches. The left ovary is very tender, and lies to the left posterior.

She was treated, and in 1892, four years afterwards, says that after treatment she was quite well till nineteen months ago, when she had diphtheria, and soon recovered; but in a month had slight uterine hæmorrhage, and pieces of skin came away, and she had pains round the abdomen. A year ago had pain round the abdomen and in her arms, and passed a lump the size of half a small orange. Six months ago had pain in the abdomen and back like inflammation, and noticed a lump in the left lower abdomen, where she has had pain ever since. The catamenia are about normal. She is very thin, sallow, and looks very ill. Rising from out of the pelvis nearly to the level of the umbilicus, occupying the left side, and extending to the right of the mid line is a firm tumour. vagina contains half an ounce of pus. secretion from the os is a slight normal mucus. The abdominal tumour is continued into Douglas pouch, where it is softer, and to the left of the uterus, which is pushed to the right by the tumour. The pus is seen to enter the vagina by a small opening through the posterior vaginal roof.

CASES FOR COMMENTARY.

1. An elderly female is suffering from strangulated femoral hernia, for which herniotomy is performed. After cutting open the sac it is found to contain a small knuckle of intestine, and a large piece of omentum, the latter being firmly adherent to the sac. How would you complete this operation? Give a detailed description.

2. A man has just received a severe blow on his perineum, followed by local swelling, and slight hæmorrhage from the urethra. Describe the probable effects of such an injury, immediate, and remote. How would you examine the case in the first instance? Give your treatment from the commencement.

3. A groom, ætat 17, was thrown from a horse; he was found two hours afterwards, quite unconscious, bleeding from the nose, face flushed, jaws clenched, pupils acting slowly to light, no paralysis, pulse 50, temperature 98°.

On the two following days the pulse was 44, and there was retention of urine. He remained otherwise in much the same condition.

On the fourth day he was found lying on his right side, his chin resting on his sternum, and his knees drawn up; he became irritable if this position was changed, and would immediately return to it. Pulse 42; temperature 97°.

Fifth day. He remains chiefly in the same position. There is now incontinence of urine. The pulse and temperature are not altered. He was delirious in the evening.

In a few days he began to improve, and soon recovered.

Explain this case. How would you treat it?
