PART 12-3 --MATERIEL REPORTS AND FORMS

12-3-1. GENERAL

This part contains specific instructions pertaining to certain forms used in merchant marine safety activities. These instructions apply specifically to certain forms and reports.

12-3-25. MEASUREMENT OF FURNACES, FORM CG-836

This Form CG-836, Measurement of Furnaces, shall be completed by the marine inspector when an inspection is performed on a fire tube boiler. This report shall be submitted to the OCMI. This report is retained in the vessel file.

12-3-30. ANNUAL INSPECTION REPORTS, FORMS CG-840A, -B, -E, -F, -FA, -H, AND -J; CG-991A AND CG-991B

These reports are in book form. The marine inspector uses the appropriate form when inspecting a vessel. The completed report shall be submitted to the OCMI prior to the issuance of Form CG-841, Certificate of Inspection. This completed report shall be retained in the vessel file at the marine inspection office where the certificate of inspection is issued to the vessel. All entries in such a report shall be made so that where there occurs a change of marine inspector during an inspection the progress which has already been made will be fully apparent.

12-3-35. CERTIFICATE OF INSPECTION, FORM CG-841

12-3-35A. Copies Required and Distribution

The certificate of inspection shall be executed in quadruplicate by the officer

in charge, marine inspection. The original shall be issued direct to the master, owners, or agent of the vessel, the first copy delivered to the collector of customs for the district in which the certificate is issued, one copy shall be forwarded to the Commandant (MVI) and one copy shall be retained in the vessel file at the issuing office. Additional copies may be obtained by the owner of an inspected vessel or his authorized agent upon proper application to the issuing OCMI, which copies shall be certified as true copies.

12-3-35B. Entries on Certificate

12-3-35B (1). Expiration date. One year from date of completion of the annual inspection.

12-3-35B (2). Vessel. The name of the vessel exactly as it appears on the vessel's official papers.

12-3-35B (3). Official number. For foreign vessels enter country of registry, if undocumented enter a hyphen, and if documented enter the official number.

12-3-35B (4). Class. Passenger, freight, barge, passenger barge, ferry, towing, tank ship, tank barge, dredge, scow, etc. This designation shall be preceded by the word "Foreign" when the certificate is issued to a foreign vessel.

12-3-35B (5). Tons. Gross and net--as they appear on the vessel's official papers.

12-3-35B (6). Length. The registered length except for motorboats which shall be measured in accordance with the
