12-3-35B (25). Data. The blank spaces provided under "Data" may be used to indicate ground tackle requirements for unclassed vessels, or for the engineering data required for vessels having more equipment installed than there is space provided for elsewhere on the certificate.

12-3-35B (26). MSTS vessels. In addition to entries required on the certificate of inspection for other vessels the following entries shall be made for MSTS vessels certificated by the Coast Guard:

(a) On the tenth line of the certificate of inspection starting with and including the line containing the word "Vessel" an asterisk shall be inserted at the word "thereunder" and in the blank space below the phrase ''total number of persons allowed" there shall be inserted an asterisk and the note "In accordance with the standards applicable to MSTS vessels."

(b) In the space provided on the certificate of inspection following the word "Class" enter the class as indicated in 12-3-35B (4) and two asterisks. Beneath the note inserted in accordance with 12-3-35B (26) (a) insert two asterisks and add the note "Naval transport, in service, civilian manned", "Naval cargo vessel, in service, civilian manned", "Naval tankship, in service, civilian manned", as appropriate.

NOTE: The term ''in service" as applied to vessels of the MSTS refers to those MSTS vessels which are manned by civil service or merchant marine crews in contra-distinction to those MSTS vessels which are manned by Naval crews and which are termed "in commission."

12-3-35C. Endorsements on Certificate

12-3-35C (1). Endorsements concerning hours of duty may be discontinued since the endorsement usually is

merely repeating a law or regulation except in cases where the endorsement is required by R.S. 4463 (46 U.S.C. 222).

12-3-35C (2). Endorsements for the carriage of deck cargo on tank barges may be discontinued.

12-3-35C (3). Endorsements for the carriage of vehicles on tank barges may be stated as:

"Permitted to carry vehicles--total wt. 18,000 lbs. per unit--axle load 6,000 lbs. per axle. Vehicles not permitted to be operated."

12-3-35C (4). Endorsement for watchman on tank barges may be discontinued because it merely repeats a regulation. Require the watchman in the crew requirements.

12-3-35C (5). Endorsements required by 46 CFR 38.01-5 and 39.01-5 shall be made as required.

12-3-35D. Crew Requirements

12-3-35D (1). In all cases where a pilot may act as master on vessels of over 150 gross tons, such pilot shall be a first-class pilot or a second-class pilot who has reached the age of 21 years.

12-3-35D (2). Watchmen shall be listed separately.

12-3-35D (3). Firemen and watertenders, either of the designations may be crossed out when only one of the ratings is required.

12-3-35D (4). For unmanned barges enter the word 'UNMANNED" on the same line and immediately after " Lifeboatmen:".

12-3-35D (5). The practices of requiring certificated engineering personnel on the certificate of river vessels and

12-3-45. TEMPORARY CERTIFICATE OF INSPECTION, FORM CG-854 Form CG-854, Temporary Certificate of Inspection, authorized by R.S. 4421 (46 U.S.C. 399), is intended to be is sued only for the purpose of permitting the immediate operation of a vessel after completion of the inspection and prior to delivery to the vessel of the Certificate of Inspection, Form CG-841. (See (See 12-3-35.) Under no circumstances shall a temporary certificate of inspection be issued for the temporary operation of a vessel which does which does not qualify for a certificate of inspection.

12-3-50. CERTIFICATE AMENDING CERTIFICATE OF INSPECTION BY CHANGING CHARACTER OF VESSEL, ROUTE, EQUIPMENT, ETC., FORM CG-858

This Form CG-858 shall be made out and copies distributed in the same manner as for a certificate of inspection, Form CG-841 (see 12-3-35). When changes in the character of the vessel, route, equipment, etc., occur during an annual inspection this form should not be executed. The period of validity shall coincide with the certificate of inspection on U. S. vessels and will be valid for one year in the case of foreign vessels. This form shall not be used as a permit to carry persons in addition to the crew on foreign vessels.

12-3-55. REPORT OF NUMBER OF PASSENGERS ALLOWED TO BE CARRIED ON VESSEL WHERE THERE IS NO INCREASE IN PASSENGER ALLOWANCE, FORM CG911

This report on Form CG-911 shall be submitted to the district commander by the OCMI upon completion of each annual inspection of a passenger vessel where there is no increase in the passenger allowance. A copy of this report shall be retained at the originating office. 12-3-60. INCREASE IN PASSENGER ALLOWANCE SUBMITTED FOR APPROVAL TO COMMANDER, COAST GUARD DISTRICT, FORM CG-912 This Form CG-912 shall be used in the same manner as Form CG-911 (see 12-3-55) except that it is only required where an increase in the passenger allowance is recommended.

12-3-65. RECORD OF INCREASE IN

NUMBER OF PASSENGERS ALLOWED TO BE CARRIED, FORM CG-913 This report shall be submitted to the Commandant upon approval of an in

creased passenger allowance. Copies shall be retained by the district commander and the cognizant OCMI.

12-3-70. RECORD OF ANNUAL INSPECTION, FORM CG-929

This form shall be submitted to the Commandant (MVI) in duplicate for each CG-841, Certificate of Inspection, or CG-989, Certificate of Examination for Foreign Passenger Vessel, that is issued. Both copies shall be stapled to the upper left hand corner of the Commandant's copy of the pertinent certificate (CG-841 or CG-989). The Form CG-929 shall be submitted for all MSTS vessels whether or not a certificate is issued and shall show whether or not a certificate is issued. The form CG-929 may be used to report a vessel placed in operation from a laid up condition.

12-3-75. RECORD OF INSPECTION OTHER THAN ANNUAL INSPECTION, FORM CG-930A

12-3-75A. When Submitted

One copy of Form CG-930A shall be submitted to the Commandant (MVI) for each inspection made except for those inspections reported on other card records, such as CG-929, Record of Annual Inspection; CG-930B, Record of Examination of Cargo Vessel to Carry Persons in Addition to Crew; CG-930C, Record of Reinspection, etc.

12-3-75B. Vessels Under Construction or Conversion

Every visit or inspection made to a vessel while the vessel is under construction, conversion, or survey conducted prior to conversion, shall be reported to the Commandant (MVI) on card record Form CG-930A. Where the visits or inspections of a vessel under construction, conversion, or survey conducted prior to conversion, are carried

out over a period of time, several examinations or inspections may be consolidated on one card record; however, such card record should not cover a period greater than one month. On the reverse side of the card record should be indicated briefly the examination or inspection performed on the vessel in order that the Commandant may be fully advised regarding the progress of the construction or conversion.

12-3-75C. Drydock Examinations Each drydock examination shall be reported to the Commandant (MVI) on card record Form CG-930A. Information concerning the condition of the underwater body of the vessel and appendages shall be included in a brief statement, as well a brief list of requirements regarding replacements or repairs considered necessary to restore the vessel to a seaworthy condition.

12-3-80. RECORD OF EXAMINATION OF CARGO VESSEL TO CARRY PERSONS IN ADDITION TO CREW, FORM CG-930B

Whenever an examination of a cargo vessel is made for determining whether or not such vessel may carry persons in addition to the crew, except where the inspection is made in conjunction with the annual inspection or the inspection is reported on Form CG-932, such inspection shall be reported to the Commandant (MVI) on card record Form CG-930A. This Form CG-930A shall be submitted in every case where the inspection is made on a foreign cargo ves sel for the purpose of determining whether or not such vessel may carry persons in addition to the crew.

12-3-85. RECORD OF REINSPECTION, FORM CG-930C

Every reinspection of a vessel shall be reported to the Commandant (MVI) on card record Form CG-930C.
